MI 8543 form

I was asked to mail in MI 8543 form to sign my state taxes because I filed with a cpa the year before. But what do i send with it. Is it the copy of my self taxes done. Signed? It was already accepted by the state. I Efiled my federal and there saying I need to send that form 2 days after it's accepted. But I downloaded it and it needs to be sent with a copy of something. Is it my state return filled out and done already?

Michigan ask that you sign this and keep it with your tax records for six years. The information on the form is electronically filed with your return, and the physical copy only needs to be maintained by you unless Michigan asks for it (which is not likely).
