Question

My 1099G Unemployment only has boxes 1-5. Box 4 show state tax withheld, but no box 10b state id number. How should I complete this information?

The boxes on my 1099G do not align with the boxes information being prompted for unemployment.  I can figure out which details to put in except for box 10b.

Since you did have state taxes withheld, that means that boxes 10a, 10b, and 11 must be completed to avoid an error condition. 

 

For box 10b, you can use the Federal ID number since no state ID number was provided on your form.
