My Michigan filing instructions say I need to sign form MI-8453. Where do I get that form?

Form MI-8453 should be in the PDF package or printed paper package of your return, once you've completed your return. Unless the State of Michigan asks for this form, which is exceedingly doubtful, it won't really matter whether you sign it or not. You just need to keep it for your records.

According to Michigan's Where do I mail the Michigan Individual Income Tax Certification for e-file MI-8453? page,

If the MI-8453 is used to sign the e-file return and the return is e-filed by an individual taxpayer, the MI-8453 should be retained by the taxpayer.

Do not mail this form to the Michigan Department of Treasury unless requested to do so.
