I am confused what to put in the boxes but on Turbo Tax its mentions Cafeteria Plan and I elected to drop the Dental Plan my work has because it was horrible and I picked up a Dental Plan on my own and now I get a $3.50 payment from my work every 2 weeks and it show up on my check pay stub as E03 CafePlan under Additional Earnings Column. Do I enter that info in one of the boxes and if so is there anything else I need to add so that it all checks out with NJ taxes? Also on my W2 Box 12a has code C and $577.68 Do I add any boxes for any of the items in Box 14 on my W2? It has SUI, SDI, Union and Health. Any help is greatly appreciated.

You should contact your employer if you have specific questions, but yes the dental premiums are most likely cafeteria- they are usually reported on your W2 Box 14. The Code C Box 12 is for Life Insurance premiums over $50,000, it is imputed income but does not have to be reported on your tax return. The other codes for Box 14 are for information only. The only one you need to indicate in TurboTax is the SDI.
