North Carolina D-400 form does not print properly

In the pdf and in the printed output, the D-400 form is askew.  Most field-names are missing, but the data is there - so you have data but no clue what Line# it belongs to.  When will this be fixed?  (assuming I cannot submit it like this)

    I didn't have any issues using W10, and the Premier Desktop software.   And Adobe Acrobat DC reader.

    Windows 10, version 21H2 (OS Build 10 0 19044) 

    Adobe Acrobat Reader DC, version [phone number removed]9, 64 bit 

     

    I have three tax returns in one pdf file:  Federal, Wisconsin, and North Carolina.

    Only the North Carolina is askew.  The other two are fine.  

     

    **  auto correct keeps removing the version numbers**

    Same OS and PDF reader as mine

     

    Strange.....IF you can, try creating a PDF of only the NC tax return, and see what happens then.

    Something may have gotten garbled in the creation of the PDF just before the NC section was created.

     

    Then do the full PDF again
