I am a 16 year old dependent child with an income that does not exceed $6000. I do not have to file a federal tax return, but do I need to file a NY state tax return? I am having trouble finding the answer, please help!

Yes, if you have income of more than $3,100 you will need to file a NY return.

I do not understand how I fill it out.

Generally, you would enter it in the W-2 section.  However, do you know why box 1 and 2 are blank?  What type of employment did you have?

What kind of income did you have?  Did you get a W2 or a 1099NEC for it?  If they took any tax withholding out of the W2 in box 2 you can file federal to get that back.  If you got a 1099NEC you have to file a federal return no matter how small the amount.

My income was $5733.75. I have a W-2, but boxes 1 and 2 are blank. I do not have a  1099NEC.

What type of income was this that boxes 1 and 2 are blank?  Did you have a job?
