Ohio RITA tax form incorrect calculation

In reviewing the local income tax form for the State of Ohio (RITA).  there appears to be an issue with Form 37,  accounting for the city credit for taxes paid to another locality.   If the W2 is entered as withheld for workplace location the form is not accounting for the income tax withheld during the year.   Lines 5 6 and 7 are to calculate a credit from the city of residence for the taxes paid to another city.   For Hilliard that credit is 100%.   However, the form is calculating a 0% credit and showing I owe 100 % of the taxes due despite actually owing nothing.

    The city of residence is Hilliard.  Is the source of line 5a from line 4a or 4b?

     

    If the entry is from 4a, for what other municipalities were you taxed?

     

    What information are you seeing in the Credit Rate Worksheet at the bottom of page two of For 37?

     

    Is the Worksheet blank or are you seeing one or more entries there?
