Hypothetical Example:

I lived in OH and worked "online" for a University in GA. I have never physically worked or lived in GA. In addition, the University provided me a W-2 not a 1099 and withheld state taxes in OH. Do I have to file state taxes for GA?

No.  If you worked from Ohio the entire time you do not have to file a Georgia tax return.  The W-2 having withheld Ohio taxes will make it easy to file.
