reporting location on OK 561 sales from Schwab 1099-b

I have a Schwab 1099-B with both short- and long-term gains. I live in OK. I did not import the 1099-B into my federal return. I manually entered what was on the 1099-B. The program is asking for either an OK location or FEIN. These mutual funds are all over the place. What am I supposed to enter there? Also, it doesn't allow for "VARIOUS" purchase date as listed on the 1099-B. That do I put there?

Anything that was from an OK sale, like a municipal bond or home, qualifies for an exclusion. Otherwise, it does not so you have to check the box that the property does not qualify.
