Schedule K-1 (Form N-20) for Hawaii

Hello, recently I received a K-1 from a fund that included Form N-20 for Hawaii.  On that form, I see the following statements in the "Credits" sections:
 
See attached Form N-756A
See attached Form N-342A
See attached Form N-312, Part II
 
However, these forms are not included in the investor packet.  I have tried contacting the fund but am not getting any response.  What should I do?

You will need to contact the issuer of the K-1 for this.
