State taxes for Ca.

I received a 1099 nec from my employer. the state ca want me to filled out form 568, I dont have a entity number from the state what must I do

    Double-check your entries to make sure you didn't select that you are an LLC.  CA Form 568 is for LLC entities.  If you don't have an LLC, you won't need the form.
