What do i send with my state tax payment to Kentucky
what paperwork do I need to send with the check
You will include a KY Voucher (Form 740-V) along with your payment (check or money order, there are instructions on the form), Make your check or money order payable to the Kentucky State Treasurer. Do not send cash. Be sure to write your name, address, Social Security number, and “2016 Form 740” on the check or money order.
