Assuming you were issued the rebate, you will receive a form 1099-Misc.  If you haven't received the form yet you can go here to view/download the form 1099-Misc and see the details on the rebate.  You will enter the amount of rebate you were issued which is reflected on that form 1099-Misc. 

 

The rebate is taxable at the federal level. So, first, you enter the form 1099-Misc information in the Federal "Wages & Income" section using the "Other Common Income" dropdown to find the 1099-Misc topic.  

 

Then, since the rebate is not taxable by AZ, you will enter the same amount in the AZ Families Rebate box which will subtract the amount from AZ taxable income.

 

If you were not issued a rebate and there is no 1099-Misc then you leave the box empty.
