What is the IT-140w state tax form. I have never had to have this

An IT-140W is a West Virginia form that is the Tax Withholding Schedule (i.e. your W-2 information for WV). It appears to be the type of form that TurboTax generates automatically so that taxpayers will not have to physically attach W-2(s).
