What subscriber number do I enter from MA form 1099-HC for a Dependent?

I am trying to enter Form 1099-HC MA Health Care Coverage:

 

1. The screen "Tell us about Jane's private health insurance" requests the subscriber number (1099-HC, Box 5) but there are two different subscriber numbers shown on the form, one in Box 5 (Myself) and another  for my dependent which is NOT in box 5 but in a different section of the form  intended for the dependent. Both Box 5 and the other non-Box 5 fields are titled "Subscriber number" on the 1099-HC but they are really "Member ID"  numbers, and the subscriber (me) is the one in Box 5. There is only one "Subscriber" on this health plan but two "members" and two different "Member ID's".

 

Should I enter the same BOX 5 Subscriber number for both individuals or should I enter two different numbers (Box 5 and the other non BOX 5) subscriber numbers?

 

2. I cannot fit the entire name of the Insurance Company in the field for Box 1, is it OK that it's chopped?

Where it asks for the subscriber number in box 5, enter the number from box 5. 

 

This is the number the policy is under. If you are asked for your dependents subscriber number, you will enter the number listed under each dependent.

 

You can abbreviate the Insurance Company name.
