your question is vague.   for what purpose?     did you get correspondence from the state?

What are you trying to do?  Are you trying to paper file your state return and are you asking what to include with the tax return?

 

Or did you receive a letter from the state requesting information after filing the return?  TurboTax does not get any information from your state after you file, so no one at TT would know what the state asked for.
