Which state forms to include if my MA return seems to print with more forms than needed

I completed my Massachusetts return and printed it for mailing. It seems to print with several additional forms at the end, so I wanted to confirm which of the following should be included. It prints with Form 1, Schedule X&Y, Schedule B, Schedule D, Schedule INC, and Schedule HC, which seem like they are required. Then comes:

 

M-2210: Do I need to include it? My tax due is about $100, so I am not sure why Turbotax included this form (showing there is no underpayment penalty) if my tax due is below $400 anyway (threshold for penalty).

 

Schedule HC Worksheet: It says "keep for your records" so I will not file this worksheet.

 

Form 1 Schedule B Line 6, Other Interest and Dividends Excluded Statement: says "attach to your return" so I will include it.

 

1040 Schedule 😧 Should I include it? I am not sure why it included this from the federal return. 

 

Or is there a place where I can print off my return from the desktop program the required forms to be mailed only without the additional worksheets and any forms not needed?

    Include Form M-2210 and the federal forms.

     

    M-2210 is necessary to show Massachusetts that you do not owe the underpayment penalty. Otherwise the state may assume that you owe a penalty.

     

    Massachusetts uses federal income so it requires certain federal forms to show how the Schedule B and Schedule D totals were calculated.

     

    You print use Print Center (upper right corner) and select All official forms required for filing. That option will print all the forms you need to send.
